APPLICATION FORM
It is a requirement of
Financial Transaction Reports Act (1988) and the Authorise Betting Operations
Act (2000) that proof of identity is required to Operate a betting account in
South Australia.
A total of 100 points is needed for this purpose and can be obtained in any
combination of following :-

A birth certificate, current passport or citizen certificate (1 only) - 70
points.

Australian driver's licence or permit containing signature - 40 points.

Councit Rates notice - 25 points.

Verified Telstra telephone book listing - 25 points.

Credit card with signature - 25 points.

Medicare card - 25 points.
All telephone bets are recorded. There are no fees to operate your account,
however a minimum deposit of $20.00 is required.
You will be allocated a Client Account Number, which will appear
on all correspondence and a PIN Number which you will be required
to memorise.
Money can be withdrawn in the following ways:-

by cash
at venues (racecourses and sporting grounds). It is important that you quote
your PIN number and present means of identification if you are not known to our
staff.

by cheque
to your home or to your postal address.

by direct debit to a nominated bank account. Cash deposits to
your account will be made within one hour of the request being made.
DEPOSITS TO YOUR ACCOUNT
Deposits can me made by cash, cheque or direct to our account at the
Commonwealth Bank. For convenience a deposit book can be posted to you which
will specifically identify your deposits made at any branch of the Commonwealth
Bank within Australia or overseas.
